2024 Membership and Insurance fees are due on or before December 31, 2023. 2024 Membership and Insurance Renewal Invoice has been sent to all members via e-mail. If a members Membership and Insurance fees are not paid by December 31, 2023, their membership will be suspended immediately on January 1, 2024 meaning that they will be unable to practice, and an administration fee for late payment in the amount of $150 will be added to their fees owing which must be paid in full prior to the lifting of a suspension.

As a reminder, the invoice that members received by email includes both the members 2024 membership renewal fee as well as their 2024 professional liability insurance renewal fees, including any additional modality coverage that is attached to their policy. This entire invoice is payable directly to CMMOTA. Payment options are listed below. If you have other policies with Dusyk & Barlow Insurance Brokers, including Accident, Auto, Home/Tenant/Condo or separate Commercial coverage, those policies will be invoiced separately.

Please be patient, as were are processing a high volume of payments.

Please note that if you are renewing later than January 8, 2024 that you will be required to fill out a new Insurance Application form with Dusyk and Barlow, which may result in further delay in your ability to practice as your insurance policy must be in place prior to your suspension being lifted.

Payment Options

E-Transfers – E-Transfers can be sent to payments@cmmota.com. Please set the password to “membership” and include your full name and CMMOTA Member # in the message (if you already know this information – if not please just include you full name).

PayPal – Payments can be sent through PayPal to payments@cmmota.com, please include your full name and CMMOTA Member #. If you would like to request a PayPal invoice please call us directly at 403-356-1160, however a PayPal invoice is not necessary to send payment through PayPal.

Credit Card – If you would like to pay by Credit Card, please call the office at 403-356-1160 during office hours, Monday to Friday, 9am to 4pm. If the phone rings busy, please leave us a voicemail and we will get back to you as soon as possible. Please note that our offices will be closed on December 30 & 31 and we will not be responding to voicemail left after 4:00 pm December 29 until we return to the office on January 2, 2024. Leaving a voice mail message after 4:00 pm December 29 will not be accepted as an excuse for not completing your renewal on time, and any resulting administration fee for late payment will be required.

Please Note – if you are making payment via eTransfer or via PayPal after 4 PM on December 29 up until 11:59 PM MT on December 31, 2023 that payment, no matter what time we process it, it will be considered to be paid on time.
